Cardiorenal Care

Protect the heart. Protect the kidneys. Start earlier.

High blood pressure and diabetes can cause progressive kidney and cardiovascular damage long before a patient feels unwell. Dialife's care pathway is designed to identify risk earlier and maintain continuity over time.

A connected cardiorenal pathway

Rather than treating blood pressure, diabetes and kidney disease as separate problems, Dialife is developing an integrated model that connects prevention, diagnosis, chronic disease management and renal care.

Who may benefit from assessment?

  • People with known high blood pressure or diabetes.
  • People with abnormal screening results or repeated elevated readings.
  • Patients with known kidney disease or a history of abnormal kidney tests.
  • People with cardiovascular risk factors who need coordinated long-term follow-up.
Screening is not a diagnosis. Abnormal screening results should be followed by appropriate clinical evaluation and, where indicated, diagnostic testing.
